Wyoming Health Council Title X Full RFA
This document contains Wyoming Health Council’s complete Request for Applications (RFA) for organizations seeking funding to provide Title X Family Planning Program services in Wyoming.
The full RFA includes program requirements, applicant eligibility, scope of services, funding information, application instructions, required documentation, evaluation criteria, and other requirements applicable to the application process.

Technical Assistance & Questions
Wyoming Health Council will provide a Technical Assistance (TA) Webinar on Monday, September 28th, 2026 from 12:00 pm - 1:00 pm MST. This webinar will support prospective applicants in understanding the Title X Request for Applications (RFA), application requirements, and submission process.
Applicants are encouraged to review the full RFA prior to participating in the webinar. Questions regarding the RFA may be submitted by email to whcteam@wyhc.org, and applicants will also have an opportunity to ask questions during the webinar.
Following the webinar, a recording and written Questions & Answers (Q&A) document will be posted here for reference.
Wyoming Health Council Title X Application
The application is due to WHC by Thursday, October 19, 2026 at 5:00 pm MST and should be submitted via the online application form. Some information will be submitted directly into the form while others will be uploaded to the form from existing documents your agency has and/or templates found in the “Templates” section of the RFA.
Please be prepared to submit the entire application form at once. Application form submissions, including all required attachments, must be received no later than 5:00 pm MST on Thursday, October 19, 2026. WHC reserves the right to request additional clarification and/or documentation during the application review process.

Applicants should review the full RFA for instructions regarding each required application material, including completion and submission requirements. All completed application materials must be submitted in PDF format through the application portal.
When preparing files for submission, include your organization name and the document name in each file name (e.g., Example Health Center - Budget Summary)

Application Preview
A PDF copy of the online application is provided for review and planning purposes. Applicants may use this document to review application questions and prepare responses in advance. The PDF is for reference only and cannot be used to submit an application. All applications must be submitted through the online application form.
FPAR Required Data Elements
The FPAR Required Data Elements are provided for applicant review and describe the data elements required for Title X Family Planning Annual Report (FPAR) reporting. Applicants should review these requirements to understand the data collection and reporting expectations associated with providing Title X services.
This RFA does not obligate WHC to fund any application or proposal. Initial and ongoing funding is contingent upon WHC receiving adequate federal Title X funding from the Office of Population Affairs (OPA)
